My Brother’s Closet Referral Form

My Brother’s Closet (MBC) provides appropriate interview attire and mentors eligible men with incomes at or below 200% of poverty, enrolled in a workforce initiative. Individuals receive coaching on professional appearance for job interviews, while receiving a free suiting.

HOURS: By Appointment Only

Please complete and submit the form below. You will then receive the secure TANF form via email (so be sure to provide an active client email address).
Once this form and the following TANF forms are complete, a MBC representative will email to schedule an appointment. You will need to provide:

  • 2 forms of identification, which will be requested through the online form (again this will be emailed after the form below is submitted).
  • Recent paystubs OR a zero income statement (included in the TANF forms)

Suiting appointments will be done with consideration for all COVID-19 Health and Safety Protocols. As such, we’re asking clients to provide measurements and preferences in advance to assist the process. Please refer to the chart on this page, or check out this helpful video, for instruction on how to take the needed measurements.
