Emergency Assistance Forms

Thank you for your interest in the Columbus Urban League’s Emergency Assistance Program. Below, please find the application as both a PDF (for printing purposes) and a Word Document (for electronic completion).

First please first read the information below to ensure you can obtain all of the necessary documentation so that our housing staff can begin to assist you immediately when they contact you.

PROGRAM ELIGIBILITY

Family Housing Stabilization Program (FHSP) – Families with children under 18

  • Maximum grant: $1000
  • MUST MEET FEDERAL GUIDELINES

Housing Assistance Program (HAP) – Individuals, no children

  • Maximum Grant: $1000
  • MUST MEET FEDERAL GUIDELINES

PROGRAM DOCUMENTATION REQUIREMENTS

  • Lease or letter of intent to rent from the landlord
  • Referral from a social service or government agency verifying state of homelessness
  • Current Proof of income for one full month (most recent 30 days)
  • Photo ID (cannot be expired)
  • Social Security Card
  • Magistrate’s notice or notice of eviction from the landlord
  • Proof of previous payments or payment agreement with landlord
  • Current proof of income for one full month (most recent 30 days)
  • Photo ID (cannot be expired)
  • Social Security Card
